The Nurse Calls The "Shots"
Dear SFMMS Parent:
We hope this will be an exciting year of learning for your child. Healthy children find learning easier and more fun. We hope that, with your help, there will be little spread of illness/disease between your child and his/her classmates.
Parents, please consider your child's well being if ANY of the following symptoms or problems are present. Please keep your child at home to help us try to prevent the spread of infection and illness.

Fever of 100.4 or higher

Earaches or sore throat
-
Diarrhea
-
Open sores on the body (scratches, cuts/bruises not included)

Red irritated eyes with yellow "crusty" discharge
-
Head lice, impetigo or scabies
-
Unexplained rash (needs doctors diagnosis)
-
Toothache
If you are concered about any of the above problems, please contact your doctor or health care provider.
Giving Medications At School 
All medications to be given at school MUST be in its original container. All prescription medication MUST have a current pharmacy label for that student.
All medication sent to be given during school hours MUST be accompanied with a permission note from the parent/legal guardian. (A form may also be obtained from Nurse Padilla-Limon.
SCHOOL PERSONNEL CANNOT GIVE MEDICATIONS FROM MEXICO OR ANY OTHER COUNTRY.
Immunization Records
By state law, NO CHILD MAY ATTEND SCHOOL UNLESS HIS/HER IMMUNIZATIONS ARE UP-TO-DATE. If you have questions about this, please contact Nurse Padilla-Limon.
Screenings
The state of Texas requires that vision, hearing, height, weight, scoliosis, and acanthosis nigricans screenings be done on children at certian grade levels. SFMMS students will be screened for scoliosis in November. You will be notified if a problem is detected.
rStudents with Chronic Illness 
If your child has been diagnosed with a chronic illness, you will be asked to provide an annual update from your doctor/health care provider. Please provide the school with a medical release from the doctor/health care provider if your child is no longer being seen for this condition.
